Why were hubs locked?

Why didn't anyone diagnosing vibration notice that?

If hubs have been locked for a long time, you'll need to inspect front axle components. Your vibration was likely from failed hub axle seals (inside the knuckle). If these get sloppy enough, they take out locking hubs, the little needle bearing inside unit bearing that stub shaft rides on, then the stub shaft and u-joint. And possibly inner axle seals depending on where you've been driving...
